Job Summary
Completes intervention training.
Conducts in-person Occupational Therapy evaluation of people living with Mild Cognitive Impairment and mild Dementia.
Co-designs 2-3 goals with the person and their care partner to participate in a nature green activity.
Creates personalized green activity plans and adapt as needed.
Conducts in-person and virtual sessions.
Provides dementia education and a site evaluation for outdoor recreational organizations.
Provides strategy training to people living with MIC and mild dementia, care partners, and outdoor professionals.
Provides monitoring and support to persons living MCI and dementia, care partners, and outdoor professionals via telephone.
Attends bi-weekly case conference meetings during the 12-week intervention.
Completes post-intervention assessments and documentation.
Qualifications
EDUCATION
Required
Master's degree in occupational therapy
Doctorate in occupational therapy
WORK EXPERIENCE
Required
2 years practice experience with people living with dementia
LICENSES AND CERTIFICATES
Required
Licensed to practice occupational therapy in New York
SKILLS
Preferred
Bilingual, speaks Spanish and English

The role would start the end of 4/5/2024-8/5/2024.
